2023 2024 WINTER SPORTS REGISTRATION

Sign up by completing the Athletic Health History form and returning to the High/Middle School NURSE'S Office.

Forms are due by Friday, November 3rd, 2023.

Winter sports are available to all 7th-12th grade students and are scheduled to begin: Monday, Nov. 13th- JV & Varsity sports Monday, Nov. 27th- Modified sports

Please send both the Physical Examination and the Athletic Health History form to our school nurses by Friday, November 3rd.

Physical Examination - Students registering for athletics must have a current physical examination form on file in the health office before the student is authorized to participate in practice/try-outs. The health appraisal form completed by the family physician should be submitted to the health office.

Please register NOW! Registrations are due no later than Friday, November 3rd. Please submit the Athletic Health History Form (linked above) and Current Physical Examination (dated 11/1/22 or more recent) to the school health office. If a physical is set for a later date, please submit the Athletic Health History form and note, physical forthcoming.
