2023 FALL SPORT REGISTRATION INFORMATION 
 
Fall sports are available to all 7th - 12th grade students and are scheduled to begin:

Saturday, August 19 - Varsity Football only 

Monday, August 21 - Junior Varsity & Varsity sports

Saturday, August 26 - Modified 7-8-9 Football only
Monday, August 28 - Modified 7-8 and 7-8-9 sports

 

All student athletes are required to submit the following mandatory forms: 

1. Physical Examination - Students registering for athletics must have a current physical examination form on file in the health office before the student is authorized to participate in practice/try-outs. The health appraisal form completed by the family physician should be submitted to the health office.
2. Athletic Health & Cardiac History Form - Recent medical update completed by parent/guardian

Registrations are due no later than June 9th. To register, simply return your Athletic Health and Cardiac History Form. Please have your Current Physical Examination (dated 8/1/22 or more recent) submitted to the appropriate school health office. If a physical is set for a later date this summer, please submit the Athletic Health and Cardiac History Form, and note the physical is forthcoming. Forms can be picked up in Physical Education class, MS or HS main offices, and also attached: 2022 Athletic Health History .docx (live.com)
