Community-Service-Hours Form (link)

(Community-service hours can be earned starting July 1st through the day all community-service-hours forms are due.  All community-service-hours forms are due by the end of the school day (2:21 PM) on the Friday after Memorial Day.  If you want earn hours before the start of the school year in September, you must inquire BEFORE school ends in June or risk not having the hours count.)
